Failure is part of the protocol.

Controlled State Requirement

Failure must produce a controlled state, not an undefined state.

A governed system must know what to do when something goes wrong. QR Protocol therefore establishes procedures for conditions including:

An action may be stopped, restricted, rerouted, escalated, recovered, placed into an exception state or referred for authorized intervention. The important requirement is that the system does something defined.

An exception is not a system error. It is a defined outcome for an interaction that cannot proceed normally, and it is governed with the same rigour as a successful path.

Stop

The interaction does not continue through the sequence.

Restrict

A narrower action is permitted in place of the requested one.

Reroute

The interaction is directed to an authorized alternative pathway.

Escalate

The condition is referred for authorized intervention.

Recover

The interaction is returned to a controlled state where recovery is defined.

Hold

The object or interaction is placed into an exception state pending resolution.

The requirement is not that nothing fails. The requirement is that failure produces a defined, recorded outcome.

When the condition passes: The exception is handled through the defined controlled outcome.
When the condition fails: Where no defined outcome exists, the condition is identified as an unresolved failure class and escalated for Protocol review.
Department performing the operation: Protocol defines the failure classes. The department holding jurisdiction over the failed operation performs the remediation.
Record / evidence required: The failure, its class and its outcome must be recorded where required.

What happens when a governed QR interaction fails?

The interaction enters a defined exception state. Protocol permits stopping, restricting, rerouting, escalating or recovering the interaction — undefined behavior and silent continuation past a failed condition are not permitted.
