Controlled State Requirement
Failure must produce a controlled state, not an undefined state.
A governed system must know what to do when something goes wrong. QR Protocol therefore establishes procedures for conditions including:

- When the condition passes
- The exception is handled through the defined controlled outcome.
- When the condition fails
- Where no defined outcome exists, the condition is identified as an unresolved failure class and escalated for Protocol review.
- Department performing the operation
- Protocol defines the failure classes. The department holding jurisdiction over the failed operation performs the remediation.

- The failure, its class and its outcome must be recorded where required.
