A record is only useful if the event can be rebuilt from it.
Audit & Recording establishes that meaningful transmissions and state-changing events create records. This document states what those records must establish.
A sufficient record establishes
- •Who or what acted during the interaction
- •What occurred during the transmission
- •When it occurred on the infrastructure timeline
- •Which Protocol governed the action
- •Which conditions were evaluated prior to activation
- •What outcome resulted from execution
Refusals carry the same evidentiary weight as approvals. A governed system that records only what succeeded cannot demonstrate that what failed was refused for a governed reason.

Protocol establishes the requirement. The authoritative operational record and its evidence are maintained by QRCodex.us. This documentation library is not a registry and holds no records.
Definition
Evidence is the retained material that allows a governed interaction to be reconstructed after it has occurred.
Governing Rule
A record is sufficient when the interaction can be reconstructed from it without relying on memory or inference.
